Working together to support people living with a long term condition
It is estimated 30% of the UK population are living with a long term condition. With an ageing population and increasing incidence of long term conditions the challenge to the NHS is set to continue. In response to this challenge the commitment by the DoH is clear: highlighted by the PSA target and the NHS and Social Care Long Term Conditions model.

Evidence of local excellence in health and social care is not hard to find. However, in order to continue this progress health and social care economies must share best practice and learning, as we implement a systemic long term conditions model.

To facilitate this process ‘Managing Long Term Conditions 2006’ will offer clinicians, managers, leaders, co-ordinators, directors and advisors an opportunity to share best practice, hear case studies, network and receive guidance and clarity.





The challenge of working in partnership across cultural and organisational boundaries
To implement a systematic long term condition model, health and social care must co-ordinate their activities to deliver integrated patient-centred services.

This national event will bring together the many health and social care cohorts to communicate and share best practice with the aim of facilitating the process of working together across cultural and organisational boundaries.

Who should attend?
Clinicians, managers, co-ordinators, leaders, advisors from primary, secondary and social care and all those involved in supporting people with long term conditions.
