The challenge of managing long term conditions
It is estimated that 30% of the UK population are living with a long term condition. With an ageing population and increasing incidence of long term conditions the challenge to the NHS is set to continue. Currently, it is estimated this challenge equates to 80 per cent of NHS costs and two-thirds of NHS activity. In response to this challenge the vision of the DH is clear: highlighted by the PSA target, the NHS and Social Care Long Term Conditions model and The White Paper ‘Our Health, Our Care, Our Say: a new
direction for community services’.

 

Working together to support people living with long term conditions
Evidence of local excellence in health and social care is not hard to find. However, in order to continue this progress health and social care economies together with the independent and 3rd sector must share best practice and learning, as we implement a truly integrated long term conditions model.

To facilitate this process ‘Managing Long Term Conditions 2007’ will offer clinicians, managers, leaders, co-ordinators, directors and advisors an opportunity to share best practice, hear case studies, network and receive guidance and clarity.

 

The challenge of working in partnership across cultural and organisational boundaries
To implement a systematic long term condition model, health, social care, the independent and the 3rd Sector must co-ordinate their activities to deliver integrated services that support people to take greater control over their lives, that are seamless, proactive and tailored to individual needs.

This national event will bring together health, social care, the independent and 3rd Sector cohorts to communicate and share best practice with the aim of facilitating the process of working together across cultural and organisational boundaries.
