7th Annual Conference of the International Network of Integrated Care

‘Making the Shift’ from hospital to community-based care

The NHS Institute for Innovation and Improvement’s Primary Care/Long Term Conditions Priority Programme has been working with five local health and social care communities in the UK to understand how to make a sustained shift in care from hospital to community-based settings that make a real difference to patient care. Drawing on the practical experiences from these field sites, this symposium session will provide a unique insight into ‘how to’ deliver the shift.

The symposium session will be led by Gary Lucking Head of Primary Care/Long Term Conditions Programme. NHS Institute for Innovation and Improvement.

The Challenge of Chronic Disease Management in Very Frail Older Persons

Many elderly persons have complex and severe acute and chronic conditions and/or disabilities leading to them being frequent and costly users of acute care. This symposium session draws on British and international experiences to explore the characteristics of the very frail older person, the nature of specialized geriatric services, and the role of primary and community care in meeting their needs.
