Simon Stevens
Former senior health policy adviser
to 10 Downing Street. Regarded as the
most influential voice in the development
of health policy at the DoH in the past
5 years where he co-authored the reform
blue print ‘The NHS Plan’.
As a senior manager in the late 1980s
and 1990s, he was Executive Director
of a health authority, General Manager
of a psychiatric hospital and Group
Manager at Guy’s & St Thomas’
Hospitals in London. Simon was recently
appointed European president for United
Health Group, the company behind the
‘EverCare’ programme. He
is also a visiting Professor of health
policy at London School of Economics.
David Nicholson CBE
NHS System Reform Leadership Group Chair
for the Department of Health, the group
created to ensure various elements of
the reform programme are co-ordinated
and to champion the implementation programme
by identifying and disseminating ideas
and approaches to implementation that
have proved successful. From the late
1980s he has been a NHS Regional Director
and Chief Executive of a number of Acute
Trusts. David is also Chief Executive
of Birmingham and Black Country SHA
which is a leading health economy in
implementing case management for complex
chronic diseases. David was awarded
the CBE in January 2004 for his services
in the NHS.
Current
situation
As many as 17.5 million adults in the
UK may be living with a chronic disease.
With an ageing population and increasing
incidence of chronic disease the burden
on the NHS is set to increase. In response
to this challenge the commitment by
the Department of Health on the future
of chronic disease management is clear;
highlighted within the NHS Improvement
Plan and the public service agreements
(PSA) recently made between the Treasury
and the DoH. Focusing on US models (Kaiser,
EverCare and Pfizer) has and will provide
invaluable experience. However the NHS
will not necessarily cope with an alien
system being bolted on. The best of
the NHS needs to be developed and integrated
with the new (and not so new) case identification
and management approaches. This conference
offers practical guidance on how to
convert the theory and experience from
both the US and the UK into the systematic
delivery of case management of complex
CDM across your economy.

service targets (PSA) have been reconfigured
to reflect evolving priorities which
include:
‘Chronic Care Management’,
which will improve health outcomes for
people with chronic conditions by providing
a personalised care plan for those most
at risk. This will reduce emergency
bed days by 5 per cent by 2008 and improve
care in primary care and community settings
for people with chronic long-term conditions’.
